ATTENTION All Teachers, Counselors, Community Leaders & Public Servants

It is time to nominate your outstanding student, group or team for our annual Southfield Youth Assistance "Youth Recognition Awards Ceremony". This is your opportunity to make a positive difference in the life of a young person, group or team. All nominees must be a resident of Southfield or Lathrup Village. Please read all the information thoroughly before making any selections. We do not want to turn away any students, however, the committee will decide the student's qualifications based on the categories described below.

1) Outstanding Community Service includes those students who have done community service beyond the school and organizational requirements. (Examples would be assisting the physically challenged, working with senior citizens, the homeless and those with special needs, or any other activity that benefits the community.) Please include all the volunteer hours.

2) Personal Achievement includes those students who have overcome personal, social, physical or emotional adversity throughout the school year and demonstrated the ability to excel above the obstacles.

3) Academic Achievement includes those students who have achieved honor roll status or have greatly improved their grades from previous school years.

Please include the past and current GPA on the nomination form. The awards ceremony will be a breakfast event on Friday, May 21, 2010 at 7:30 a.m. at Vassel's Banquet Hall in Southfield. We will notify you if the youth you nominated will be recognized. Please make copies of the forms as needed. We look forward to hearing from you. If you have any questions, please feel free to contact the Southfield Youth Assistance Office @ 248-746-7658.

Sincerely,

Ollie Colvard, Chairman
